Mokokchung, 5 July (MTNews): The Chief Medical Officer Mokokchung has informed all the state government employees under Mokokchung Districts who are yet to apply for CMHIS registration to update their mobile numbers. The employees have also been informed for addition of dependents as well as for card generation as mentioned on Annexure (refer table below).
It was also informed that each District Office should arrange two employees for SPOC and BIS Operator for hands-on training at the respective department and Offices. “All employees and dependants should produce xerox copy of PMJAY Card and CMHIS Self Certification with duly signed if already registered and PMJAY Card generated,” it was further informed.
In case of any medical emergency, card generation can be done on an urgent basis at CMO Office after 02:00 PM daily, it was informed.
