Mokokchung, June 23 (MTNews): In pursuance of the letter from the Ministry of Social Justice & Empowerment, Department of Empowerment of Persons with Disabilities (Divyangjan), an Assessment Camp for distribution of aid and assistive devices by ALIMCO under Rashtriya Vayoshree Yojna (for Senior Citizens) and Assistance to Disabled Persons for Purchase (ADIP) Schemes is scheduled to be held on the following dates and venues as mentioned:
5th July 2022, IMDH at 10:00 AM
6th July, CHC Mangkolemba at 10:00AM
7th July, CHC Changtongya at 10:00 AM
8th July, CHC Tuli at 10:00 AM
Further, the following documents are to be produced mandatorily by the beneficiaries on the day of Assessment Camp.
For Rashtriya Vayoshree Yojna (Senior Citizens):
1. Passport size photo,
2. Copy of Age & Address proof (AADHAR Card/Voter ID) or any valid Govt. ID Card,
3. An income Certificate (Equal to or below Rs. 15000/- per month) issued by appropriate authority (Any Gazetted Officer),
4. Photo copy of proof of receiving Old Age Pension under IGNOAPS or any other State Govt. Pension for BPL categories.
For Assistance to Disabled Persons for Purchase (ADIP) Scheme:
1. Passport size photo showing disability,
2. Photocopy of Address proof (AADHAR Card/Voter ID) or any other valid Govt. ID Card,
3. An income Certificate (Equal to or below Rs. 15000/- per month) issued by appropriate authority (Any Gazetted Officer),
4. Disability Certificate having disability 40% or above.
