The Office of the Deputy Commissioner, Mokokchung, has informed the public about the Dr Ambedkar Foundation’s Medical Aid Scheme, which provides financial assistance to eligible patients from Scheduled Caste and Scheduled Tribe communities suffering from life-threatening ailments.
The scheme covers patients requiring treatment for heart surgery, kidney surgery and dialysis, cancer surgery, chemotherapy and radiotherapy, brain surgery, kidney or other organ transplantation, spinal surgery and other life-threatening diseases.
Eligible applicants must belong to families with an annual income below Rs 5 lakh and should not be covered under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana or the Chief Minister’s Health Insurance Scheme.
Applicants are required to submit the prescribed application form along with self-certified copies of their caste or tribe certificate, income certificate issued by the Deputy Commissioner’s office, ration card or Aadhaar card, and an estimated cost of the surgery, transplant, dialysis or chemotherapy certified by the Medical Superintendent of the hospital.
Applications should be submitted to the Director, Dr Ambedkar Foundation, 15 Janpath, New Delhi, at least 15 days before the proposed date of treatment.
The approved assistance will be released directly to the concerned hospital and not to the applicant. The maximum assistance available under the scheme is:
•Heart surgery: Rs 1.50 lakh
•Kidney surgery/dialysis: Rs 3.50 lakh
•Cancer surgery/chemotherapy/radiotherapy: Rs 2.50 lakh
•Brain surgery: Rs 2 lakh
•Kidney/organ transplant: Rs 3.50 lakh
•Spinal surgery: Rs 1.50 lakh
•Other life-threatening diseases: Rs 2 lakh
